Practice Nurse

Job Purpose The Practice Nurse will provide high-quality, evidence-based nursing care to the registered population of Market Weighton Surgery. The post holder will be responsible for delivering preventative care, health promotion activities, chronic disease management and treatment services, whilst supporting the ongoing development of nursing services across the practice. The role requires autonomous clinical decision-making, specialist knowledge of long-term condition management and the ability to work collaboratively within a multidisciplinary primary care team. The post holder will support Yorkshire Health Partners' commitment to delivering safe, effective, patient-centred care and contribute to service improvement, quality initiatives and the achievement of contractual and clinical targets. Key Responsibilities Clinical Practice The post holder will: • Deliver safe, effective and evidence-based nursing care to patients across the lifespan. • Assess, plan, implement and evaluate programmes of care for individuals and groups of patients. • Manage and review patients with long-term conditions including: • Diabetes • COPD • Asthma • Hypertension • Coronary Heart Disease (CHD) • Deliver nurse-led chronic disease clinics and undertake call and recall activities. • Undertake cervical cytology screening. • Deliver childhood, adult and travel immunisations and vaccinations in accordance with national guidance and Patient Group Directions (PGDs). • Undertake health promotion and preventative healthcare interventions. • Perform and interpret clinical procedures including: • Spirometry • ECGs • Wound care • Venepuncture • Health assessments • Review medications in line with national and local protocols and support patient adherence to treatment plans. • Respond appropriately to urgent and emergency situations and escalate concerns where necessary. • Support family planning and womens health services. Patient Care and Communication The post holder will: • Communicate effectively with patients, carers and colleagues. • Provide specialist nursing advice and education to patients and families. • Support patients in self-management and lifestyle modification. • Demonstrate empathy, compassion and professionalism when delivering care. • Act as a patient advocate and uphold patient choice, dignity and respect. Leadership and Team Working The post holder will: • Act as a positive role model for clinical staff • Support the Lead Practice Nurse in the delivery of nursing services and practice priorities. • Participate in clinical supervision, team meetings and service development initiatives. [ • Mentor, supervise and support junior staff, students and healthcare assistants. • Contribute to appraisals, staff development and competency assessments where appropriate. • Work collaboratively across multidisciplinary teams and external healthcare organisations. Quality, Governance and Service Development The post holder will: • Maintain accurate and contemporaneous patient records. • Ensure compliance with NMC standards, CQC requirements and local policies. • Participate in clinical audit, quality improvement and research activities. • Contribute to the development and implementation of clinical policies, pathways and protocols. • Support achievement of Quality and Outcomes Framework (QOF) and other contractual targets. • Maintain effective infection prevention and control standards.
