Job description
Job Summary:
• Plan and provide appropriate nursing care to patients of all ages, in conjunction with other nurses and medical staff
• Carry out investigations, assess and deliver evidence based practice in accordance with practice policy and protocols
• Offer advice and health education to ensure disease treatment is complimented by health promotion
• Maintain a safe and effective practice at all times and bring any deficits in care to the attention of the Nurse Lead so that they can be addressed
• Participate in the development of practice protocols to advance nursing roles, enhance safety and improve patient care
• Mentor and supervise HCAs
• Provide support to the wider MDT team to suppot high quality, patient-centred care to patients
Clinical Responsibilities
• To assess, plan, implement and evaluate the needs of patients to include treatment, preventative care, appropriate screening and education.
• Undertake patient consultations, carry out physcial examinations, investigatory procedures and collection of specimens
• Manage annual reviews of patients with long-term conditions (where appropriately trained) and undertake interim support to ensure that the condition is well controlled
• Provide wound management including both chronic and postoperative wounds
• Administer adult and childhood immunisations, and other injections under Patient Group Directions ensuring safe storage, rotation and disposal of vaccines and
• Liaise with other health professionals both within the practice and in other organisations to ensure integrated care
• Ensure that current evidence based approaches to patient care are standard practice
• Undertake diagnostic, health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy
• Provide contraceptive advice and cervical screening
• To provide health promotion advice
• To provide travel health advice and immunisations
• Undertake annual and interim chronic disease reviews in accordance with patient need and practice recall procedure
• Provide first aid and management of emergencies
• Undertake annual training in anaphylaxis and resuscitation techniques
• Ordering childhood immunisations, ensuring adequate supplies and stock rotation, completing spreadsheets and monitoring fridge temperatures
• Provide dementia screening
• Record Doppler studies, measurement and fitting of compression hosiery
