Job description

Clinical Nursing Practice and Care Management Manage the nursing care for individuals and groups within the practice population, including direct access to specialist nursing care for patients within the practice setting. Liaise as appropriate with specialist nurses. Undertake diagnostic, health surveillance, health screening, and therapeutic interventions within a broader health promotion/public health strategy which includes responsibility for health promotion information in the practice. Manage programmes of care for patients with acute and chronic disease by planning, providing and evaluating care including audit, recall and evidence based practice. Work with and participate in the development of protocols, policies and standards as agreed within the practice. Contribute to the identification of priorities and goals for the nursing team in consultation with other members of the practice team. Participate in prescribing according to agreed patient group directives (PGDs). Provide nursing expertise to the practice team acting as a source of expert advice on nursing and health issues. Other duties as required by Partners/ GPs & Practice Manager.
