Job description
Work as an accountable and autonomous practitioner working alongside the community Clinical Nurse Specialists in the day to day running of the specialist palliative care service, working within a defined caseload.
Provide specialist palliative care advice and support to patients and carers affected by a diagnosis of advanced cancer or other life-threatening long-term conditions, to address complex patient/carers needs in the promotion of quality of life and the experience of death.
Provide specialist support and education on all aspects of palliative care to members of the multi-professional team in a variety of settings.
Assess requirements for bereavement care and support for those carers and family members known to the service.
To work in collaboration with Macmillan Cancer Support.
Undertake shifts on the Single Point of Access line, to provide support and guidance for those with urgent palliative needs.
