Primary Care Paramedic

Job Purpose Provide autonomous clinical care across the PCN through home visiting, care home support and integrated neighbourhood working, improving access, reducing avoidable admissions and delivering proactive care in line with the 2026/27 Network Contract DES. Key Duties Undertake acute home visits. Assess, diagnose and manage patients within scope of practice. Lead clinical support for allocated care homes including ward rounds. Support frailty, anticipatory care planning and end of life care. Participate in MDTs Maintain accurate records and clinical governance. Support quality improvement , audit and education.
