This would ideally be a six session post. Key Responsibilities: Provide effective medical care to patients.. Conduct patient consultations via telephone, face-to-face, and occasional home visits where clinically appropriate. Manage long-term conditions effectively, ensuring continuity and a proactive approach to patient care. Assess and treat patients with undifferentiated and undiagnosed health problems, using sound clinical judgement. Prescribe generically and in accordance with both local and national prescribing guidelines. Maintain clear, accurate, and contemporaneous clinical records in line with professional standards, practice policy, and legal requirements. Process administrative tasks in a timely and efficient manner, including referrals, pathology results, repeat prescriptions, and other documentation. Adhere to evidence-based best practice as recommended through national clinical guidelines and the audit process. Work collaboratively with other members of the primary care and community services teams to ensure coordinated, holistic care.
