Job description
The successful candidate will:
• Provide high-quality, patient-centred primary care services to the practice population.
• Undertake face-to-face, telephone and online consultations.
• Assess, diagnose and manage a wide range of acute and chronic medical conditions.
• Prescribe safely and in accordance with current clinical guidelines.
• Review and action clinical correspondence, test results and medication requests.
• Make appropriate referrals to secondary care and community services.
• Maintain accurate and contemporaneous clinical records using EMIS.
• Work collaboratively within a supportive multidisciplinary team.
• Participate in clinical meetings, quality improvement initiatives and service development.
• Support continuity of care and promote positive patient outcomes.
• Maintain professional development and participate in appraisal and revalidation processes.
• Contribute to teaching and mentoring activities where appropriate as the practice develops its role as a teaching practice.
