Job description

Clinical Responsibilities The post-holder will: Deliver safe, effective and evidence-based clinical care. Maintain accurate and contemporaneous medical records. Develop personalised management plans with patients. Promote health education, disease prevention and self-management. Ensure continuity of care through appropriate follow-up. Adhere to NHS, GMC and practice clinical standards and guidelines. Follow safeguarding, confidentiality, GDPR and information governance requirements. Practice Responsibilities The successful applicant will be expected to: Work collaboratively with GPs, nurses, pharmacists and the wider multidisciplinary team. Participate in practice meetings, MDT discussions, clinical governance and significant event reviews. Contribute to the Quality and Outcomes Framework (QOF) and enhanced services. Support quality improvement initiatives and CQC requirements. Maintain annual appraisal and mandatory training requirements. Contribute to the education and supervision of GP Registrars and other learners attached to the practice where appropriate.
