Job Title: School Nurse
Band:6
Department: Children and Young Peoples Services
Location and mobility: Your normal place of work will be in one of the Enfield Community Bases. However, you may be required to work at any other locations of the Trusts interests, including travelling to appropriate meetings, outside of the borough as required.
Accountable to: Service Manager Children and Young Peoples Services.
Responsible to: Team Leader
Responsible for: Members of the skill mix team
Working Relationships
Internal: 0-19 team, HR, Safeguarding
External: Social Services, GPs and liaison with the multi-disciplinary team
Job Summary:
The post holder will be a qualified School Nurse. To work as part of a team delivering the Healthy Child Programme (HCP) to the school aged population in Enfield. The School Nurse will work in partnership with Children and Young Peoples team, service users, professional networks and stakeholders to provide Commissioned Services to meet Key Performance Indicators. Ensuring the best possible clinical outcomes by using up-to- date skills and adhering to evidence based policies and procedures.
Main Tasks and Responsibilities
1. To work in partnership with team members, service users and all stake holders in promoting the health and well-being of Children, Young People aged 5-19 years and their families. This will be achieved through effective delegation, work load management, monitoring of care standards, leadership and the performance management of team members.
2. To identify and monitor health needs of a given population using profiling tools and facilitate the implementation of evidence based care. The practitioner will work in line with the Healthy Child Programme and all policies that promote and safeguard children 5-19 years and their families.
3. To actively promote immunisation programme.
4. Support Clinical Service Lead and team to achieve universal and targeted commissioned services.
5. To manage and prioritise workload according to the identified health needs within a defined caseload ensuring the effective use of resources.
6. To Review regularly with the Team Leader and Child Protection Supervisor in supervision families causing concern, record agreed action and follow through according to Trust and Safeguarding policies.
7. To organise, develop and participate in public health activities including health promotion with individuals and groups.
8. To work within the policies and guidelines set out by the Trust and NMC- Code of Professional Conduct.
9. To take responsibility for Nurse Prescribing in line with National and local guidelines.
10. To be responsible for the supervision and delegation of duties of support staff.
11. To keep contemporaneous notes and accurate records and maintain client confidentiality according to NMC and Trust guidelines.
12. To undertake such other duties as may be reasonably required from time to time as are consistent with the responsibilities of the post.
13. To participate in research/audit and statistical data which are approved and agreed.
14. To contribute to policies and the development and implementation of care pathways.
15. Promotes a culture that is committed to innovation and quality improvement.
16. Demonstrate a commitment to providing high standards of evidence based care to children and families.
17. Ability to accept challenge and to challenge others in a constructive and considered manner.
18. Able to demonstrate positive leadership qualities as part of an empowered team.
Communication
Dealing with sensitive or contentious information
To participate in identified meetings such as case conferences, core group meetings and other meetings as appropriate.
To participate in multi-agency working within the 0-19 services.
To be conversant with the Trusts Child Protection Procedures referring any concerns as appropriate.
Patient/Customer care
To undertake home visits/school visits
To identify children who may require more support and refer to relevant professionals to meet the needs of children and families.
To identify the Public Health needs of individuals and communities; and working within specified agreed national and local targets.
To contribute to the planning and implementing of effective health promotion and health education programmes for client group within communities to support Public Health Outcomes.
People (HR) Management
Day to day supervision of staff/local induction
Line management responsibility of junior staff ensuring they have an annual appraisal and supervision.
Information Management
Regular requirement to write reports for case conferences and other meetings as appropriate.
Responsible for the maintenance of own record keeping on Patient record information system (RIO) and to adhere to record keeping standards in accordance withTrust guidelines and the NMC.
To have excellent computer skills and be willing to undertake information Technology training to develop skills in electronic record keeping and data collection.
Policy Development
To participate and contribute to research, audit and evaluation of new ways of working.
To adhere to all Trust policies, procedural guidelines and current legislation.
Service Development
To participate and contribute to developing practices and protocols
Resource Management
To ensure the correct use and storage of resources.
Systems and Equipment
To maintain equipment and report to senior staff when there is any malfunction
To be aware of Health and Safety hazards in the working environment and report them to senior member of staff. To be aware of Trust policies and procedures with regard to Health and Safety.
