1.Effectively manage and action all referrals received in a timely manner.
2.Ensure accurate and timely recording of all patient, carer, and professional contacts within SystmOne.
3.Deliver hands-on, patient-centred care in accordance with agreed care plans.
4.Take a central role in the day-to-day co-ordination of patient contact using our clinical noting system ensuring deadlines are met.
5.Coordinate seamless patient transitions from admission through to discharge within the Virtual Ward.
6.Maintain strong working relationships with internal teams, external partners, and relevant agencies.
7.Identify patients requiring complex discharge planning at an early stage and facilitate appropriate pathways.
8.Develop and maintain an advanced working knowledge of SystmOne, supporting system improvements, reporting, and efficiency initiatives (including KPIs and activity reports).
9.Support audit processes and act as a Tendable champion for the service.
10.Manage general enquiries, escalating clinical or operational concerns appropriately.
11.Communicate effectively using range of methods (such as telephone and virtual platforms) compassionately with patients, families, carers, and professionals.
12.Promote a positive, collaborative team culture that encourages continuous learning and service improvement.
13.To carry out noncomplex telephone contact with patients and families. This may include bereavement calls.
14.Using various methods of observation, report any change in the patients condition to the appropriate Health Care Professional.
15.Support in monitoring the lone working within the team.
16.Undertake additional duties across the clinical areas of the hospice based on service demands to broaden and maintain skills and knowledge.
17.Assist the Clinical Nurse Specialists or Specialist Paramedics with joint visits as necessary and carry out independent home visits if required.
18.To recognise and manage the potentially distressing effects of working with terminal illness, maintaining personal resilience while accessing the support, guidance, and resources available through the hospice.
19.Provide support to help patients achieve goals, adapt to illness, and develop self-management strategies.
20.Undertake patient assessments within scope of competence to support safe care delivery.
21.Maintain a safe, caring, and supportive environment for patients and their families/carers.
22.Maintain patient confidentiality at all times and ensure sensitive information is handled appropriately in accordance with organisational policies and legal requirements.
23.Attend mandatory training, including core principles of palliative and end-of-life care.
24.Digital tools may be used to support patient monitoring, and the post holder will assist patients and carers in using this technology effectively.
