Senior Occupational Therapist

The post holder will: Work as an autonomous practitioner within an integrated team providing timely and expert professional assessment and treatment interventions to patients. Provide case management for a designated caseload of patients who have immediate and / or complex care needs. Triage patients to ensure prioritisation of clinical need. Develop and co-ordinate individualised rehabilitation plans in close liaison with the team, the patient and their carers as well as all other services. Be responsible for the timely assessment and treatment of patients who have urgent and / or complex Occupational Therapy needs Co-ordinate appropriate interdisciplinary, patient centred care that promotes independence for patients on their caseload.
