Job SummaryWorking with the guidance of a clinician and strictly
in accordance with policies, procedures, guidelines and protocols. The
healthcare assistant will have a specific focus on the delivery of Annual
Health Checks for those with Severe Mental Illness - SMI.
Main Duties and
Responsibilities:
• Annual Health Checks for patients with SMI
• Other NHS Health Checks
• Measuring and recording:
• Blood pressure
• Pulse rate
• Height and weight
• Body mass index
• Point Of Care Testing Blood Tests
• Cleaning and de-sanitising
and maintenance of equipment
• Supplies,
equipment re-stocking and stock rotation
• Preparing and
maintaining environments and equipment before, during and after patient care
interventions.
• Assisting in the
assessment and surveillance of patients health and well-being
• Raise awareness
of health and well-being and how it can be promoted
• Assisting with
the collection and collation of data related to health and well-being
• Completing
administrative tasks in line with the role including contacting patients over
the phone to complete health checks
• Any other duties
deemed appropriate for the role by the responsible clinician
Administration
• Record information and activities undertaken with patients and
clinicians in an accurate and timely fashion using manual or computer systems
as appropriate
• Record information accurately and in a timely fashion using Excel
spreadsheets
• Maintain confidentiality with regards to information relating to
patients, relatives, staff and practice.
Other duties
• Practice in accordance with agreed standards of
care and always ensure own actions are consistent with clinical governance
systems
• Alert other team members to issues of quality
and risk in the care of patients
• Understand and follow
the Alliance policies and procedures.

appropriate, changed by discussion with the post holder to ensure the provision
of effective and efficient services.
