Job description

The post holder will: - Provide specialist dietetic assessment and intervention for adults with learning disabilities, using a person-centred and evidence-based approach - Manage a defined caseload with increasing autonomy, prioritising referrals based on clinical need and risk - Work collaboratively within the multidisciplinary team to contribute to assessments, care planning, and risk management - Deliver individual and group-based interventions, including weight management and nutritional support programmes - Identify and manage risks associated with malnutrition, obesity, and complex physical health conditions - Support individuals, families, and carers to access appropriate health services and make informed choices about nutrition - Develop and deliver accessible nutritional information and resources tailored to the needs of people with learning disabilities - Provide training and advice to MDT colleagues, carers, and support staff - Maintain accurate clinical records and contribute to service data collection and performance monitoring - Participate in clinical supervision, service development, audit, and quality improvement initiatives The post holder will work across community settings and contribute to improving health outcomes and reducing health inequalities for people with learning disabilities.
