Job description

To deliver high quality care to patients / service users in the community and inpatient settings. To undertake holistic mental health assessments, to act as care-coordinator for relevant patients when appropriate as part of a multi-disciplinary team. To assess, plan and implement treatment programmes and take responsibility to ensure this is evaluated and amended as appropriate.To take the lead with patient / service user management under direction of Team Leader. To provide a comprehensive education and advisory service in own specialism to patients / service users and their carers promoting independence and self-management techniques. To work at all times within the Codes of Professional Conduct. To encourage and promote flexibility and innovation within the team to meet the needs of the caseload population. To facilitate Public Health Initiatives by educating patient / service user on their health and well-being to promote independence. To help facilitate timely discharge and prevent inappropriate hospital admissions. To act as role model to all team members. Maintain accurate and contemporaneous documentation to ensure timely, effective communication and data collection. Ensure that patient / service user documentation demonstrates clearly the organisational Guidelines for Records and Record Keeping. Be personally accountable for maintaining skills, knowledge and competence in the use of RiO. Be aware of the Data Protection Act to ensure appropriate action is taken to ensure confidentiality and the protection of patient /service user identifiable information at all times. Please note that this post does not meet the pay level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa. Applicants requiring a Skilled Worker Visa can determine the likelihood of obtaining a Certificate of Sponsorship against the relevant criteria here https://www.gov.uk/skilled-worker-visa Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. A full UK driving license and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
