Job description

The post holder will provide specialist care for patients with Parkinsons disease and provide support and advice to the wider MDT. • To manage an active caseload of people with Parkinsons and to work with patients and their carers to provide interventions and to develop self-management plans for Parkinsons to optimise health and quality of life and reduce avoidable hospital admission. • To work with in collaboration with patients and their carers, GP & Hospital Consultants, to achieve optimum therapeutic benefit from medication and treatment regimen. • To support the multidisciplinary team to provide specialist care to patients with Parkinsons disease. • To develop and deliver education and training relating to Parkinsons Disease for the MDT, for other health and social care professionals and for people affected by Parkinsons. • To share expertise which is relevant to other long-term neurological conditions with other members of the MDT where appropriate. • Be an integral part of the Community Stroke and Neuro Rehabilitation Team and Neurological Conditions Specialist Practitioner team. • The post holder will maintain a clinical caseload for people with Parkinsons Disease.
