The Stop Smoking Practitioner within One You Kent delivers high-quality, evidence-based stop smoking support to residents across Kent, with a strong focus on reducing health inequalities and improving population health outcomes. The role provides targeted interventions for priority groups, including individuals living in areas of deprivation, people with mental health needs, young people, and those with long-term conditions or recently discharged from hospital. Working across community, healthcare, and outreach settings, the post holder supports behaviour change, prevention, and early intervention as part of the wider One You Kent agenda. They deliver tailored smoking cessation interventions across one-to-one, group, drop-in, telephone, and digital formats, ensuring clients are fully informed and supported to make positive, sustainable choices about their quit journey. The role also involves caseload management, accurate data recording, and active engagement and recruitment through outreach and community activity. The practitioner works collaboratively with health, social care, and community partners to embed stop smoking support within wider care pathways, contributing to peer support and relapse prevention initiatives. They deliver education on tobacco dependence, cessation methods, behaviour change, and harm reduction, while supporting individuals to set realistic and achievable goals. In addition, the role requires up-to-date knowledge of tobacco control policy and evidence-based practice, active participation in meetings, supervision, and training, and contribution to service improvement. Strong communication, relationship-building, and a compassionate, inclusive, and quality-driven approach are essential to effectively support clients and work collaboratively with colleagues and stakeholders.
