Stroke Co-ordinator

The post holder will be expected to carry out assessment and treatment of stroke patients predominantly in their own homes, within Gloucestershire. The post holder will provide a point of contact for a caseload of stroke patients within the community following their discharge home until 6 months post-discharge. The post holder will have responsibility for rehabilitation assistants, providing leadership, clinical support and training as appropriate. The post holder will work as a member of a multidisciplinary team and will act as a resource to other healthcare professionals requiring specialist advice.
