• Undertake clinical triage of therapy referrals made to the Referral Hub deciding on the correct care pathway for that patient • Carry clinical caseload to support community rehab eg Long covid rehab, fall service etc • To work collaboratively within a multidisciplinary team • Work in partnership with Referral Hub staff and management to ensure that patients are allocated to the correct team in a timely way • Liaison with acute hospitals, local authorities and other stakeholders to obtain information and resolve queries • Participate in the ongoing monitoring and evaluation of the therapy triage, ensuring that all required data is collected, undertaking audit as required. • To provide an evidence-based assessment, treatment and advisory service to clients with a variety of conditions.
