Job description

1. ROLES AND RESPONSIBILITIES To use knowledge and expertise to accurately assess patients individual needs, managing their care appropriately within the specifications of practice protocols, guidelines and Patient Group Directions. Identify and manage as appropriate patients at risk of developing long-term conditions. Support patients to adopt health promotion strategies which promote healthy lifestyles and encourage principles of self care. Deliver opportunistic health promotion. Assess and manage patients presenting for wound care. Provide a minor injury service referring to other clinicians or agencies as appropriate within the scope of competence. Participate in vaccination programmes for children and adults. Administer routine injections e.g. Zoladex, Vit B12. Promote and deliver evidence-based care for patients presenting with aural conditions. Assist in minor surgery sessions. Provide general treatment services e.g. phlebotomy, ECG, swabs, urinalysis. Act as chaperone. Ensure safe storage, rotation and disposal of vaccines and drugs. Apply infection control measures within the practice according to local and national guidelines. Understand and apply policy that supports the identification of vulnerable and abused children and adults, being aware of statutory procedure and local guidance. Participate in audit as a means of evaluating the quality of work of self and the team, implementing improvements where required. Participates in QOF and advanced service activities. Produce accurate, contemporaneous and complete records of patient consultations, consistent with NMC and practice guidelines. Mentor, support and teach other members of the primary health care team e.g. HCAs and student nurses. Practices in accordance with the NMC code of professional conduct. Takes responsibility in agreement with the Nurse Manager for own personal and professional development.
