Urgent Care Practitioner for Mental Health Liaison

The role of the Urgent Care Practitioner is to prioritise and respond to the needs of service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. The post holder will carry out psychosocial assessments, including risk assessment and formulate clinically appropriate onward plans of care, providing specialist mental health advice, support and interventions to staff, carers and patients and other agencies.
