Job description

We are currently recruiting for a Young Person Support Worker to join a dedicated service in Lewisham, supporting young people who have recently left the care system and are transitioning into independent living. London £15.00 per hour + Holiday Pay Ongoing Contract Full-Time Hours Available This is a rewarding opportunity to make a genuine difference by helping young people develop the skills, confidence, and resilience needed to live independently and achieve positive outcomes. Key Responsibilities: Manage and support a caseload of young people aged 16-25 Assist clients with developing independent living skills, including budgeting, cooking, tenancy management, and accessing education or employment Complete support plans, risk assessments, and regular keywork sessions Promote positive wellbeing, safeguarding, and personal development Support young people to access benefits, healthcare, training, and community services Maintain accurate case notes and ensure all records are up to date Work in partnership with local authorities, social workers, and external agencies Requirements: Previous experience supporting young people, care leavers, or vulnerable adults Knowledge of safeguarding and risk management procedures Experience completing support plans and maintaining case records Strong communication and relationship-building skills Ability to work independently and manage a varied caseload If you have experience supporting young people and are passionate about helping care leavers build successful and independent futures, we would love to hear from you.
