Do you want to help people stay independent, healthy and connected to their communities.
Due to the expansion of our successful service, we are looking for caring, motivated and proactive individuals to join our growing community-based team.
This exciting role focuses on supporting adults with mental health, physical health conditions and long-term health needs to live independently in their own homes and remain active within their communities.
Our approach is centred on prevention, early intervention and promoting wellbeing, helping people maintain their independence and reduce the need for more intensive support services.
As a Support Worker, you will work closely with individuals, their families and partner organisations to provide person-centred support that builds confidence, resilience and practical life skills. You will empower people to make informed choices, achieve personal goals and retain control over their daily lives.
The role encourages individuals to access local services, build social connections and take part in activities that promote health, wellbeing and community involvement.
You will contribute to the development and review of person-centred support plans and risk assessments, maintain accurate records, monitor outcomes and ensure individuals are safeguarded at all times. The role requires sound judgement, the ability to work independently and a commitment to delivering high-quality support that focuses on strengths, aspirations and positive outcomes.
We are looking for people who:
• Are passionate about helping people live independently and achieve their goals.
• Have a compassionate, strengths-based and person-centred approach.
• Can build positive relationships with individuals, families and professionals.
• Are confident working independently and responding to changing circumstances.
• Understand the importance of safeguarding, wellbeing and inclusion.
• Are committed to ongoing learning and professional development.
What we offer:
• Comprehensive induction and training.
• Ongoing development opportunities.
• Regular supervision and support.
• The opportunity to be part of an innovative and expanding preventative service.
Join us and help people maintain their independence, improve their wellbeing and live the lives they choose.
This role is suitable as a job share opportunity.
As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C2: Can express themselves spontaneously at length with a natural conversational flow, avoiding or backtracking around any difficulty so smoothly that the person with whom they are conversing is hardly aware of it.
City of York Council is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all employees and volunteers to share this commitment. We require the successful candidate to undertake an enhanced Criminal Record check via the Disclosure and Barring Service.
The Council is committed to investing in the talent and wellbeing of our staff and can offer a generous annual leave allowance, access to the Local Government Pension Scheme, comprehensive Health & Wellbeing initiatives and a reward package which recognises your contribution. We can offer flexible working practices, community engagement and volunteer activities and, as part of our commitment to you, promote continuous Learning & Development. In addition we also offer a growing range of discounts, rewards and savings.
