Advanced Clinical Practitioner (ACP) - Selly Oak Health Centre

Role Duties Clinical Practice Independently assess, examine, diagnose, and manage patients presenting with acute illness and acute exacerbations of chronic disease. • Provide expert management of conditions including: • COPD and asthma exacerbations • Heart failure decompensation • Diabetes complications • Frailty and complex multimorbidity • Acute infections in patients with chronic disease • Chronic pain flare ups • Hypertension and cardiovascular instability • Request, interpret, and act upon diagnostic investigations. • Prescribe medications independently within scope of practice. • Develop and implement evidence based management plans. • Identify deteriorating patients and escalate appropriately. • Deliver same day urgent care clinics and home visit assessments where required.
