Job description
• Undertake structured medication reviews (SMRs) for patients with complex polypharmacy, long-term conditions, frailty, or high-risk medicines.
• Support safe and effective repeat prescribing systems within practices.
• Provide advice and support on medicines management, prescribing queries, and cost-effective prescribing.
• Contribute to the management of long-term conditions by optimising medicines use and supporting self-care.
• Conduct medicines reconciliation following hospital discharge and ensure appropriate follow-up.
• Liaise with community pharmacists, hospitals, and other providers to ensure continuity of care.
• Review prescribing in line with local and national guidelines to ensure safe, effective, and efficient use of NHS resources.
• Support practices to meet QOF, IIF, DES, and other contractual requirements through high-quality prescribing.
• Provide support and advice to GPs, nurses, and ARRS staff on medicines-related issues.
• Attend PCN and practice meetings, contributing to service development.
• Engage with population health management initiatives and help address health inequalities through medicines optimisation.
