Job description
As a Vaccinator you will work as part of a team to provide
a caring, quality service to patients during the delivery of vaccinations
across the lifespan, primarily COVID-19 and Influenza vaccines, to the patient
population defined within the outreach service. This primarily covers people
who are housebound or reside in care homes, but it may include community events
or other community locations. At all times you will work within your own
competence and use your own judgement, knowledge and common sense in your
day-to-day activities.
You will
undertake activities such as clinical assessment, preparation and drawing up of
the vaccine from multi-dose vials and post vaccine observation support. In
particular, the post holder will be responsible for:
Conducting
clinical assessments.
The
review of complex medical histories and potential adverse reactions.
Preparation
of the vaccine prior to administration.
Safe
vaccination of patients across Banes, Swindon and Wiltshire in a variety of
settings.
Ensuring
the best possible clinical outcomes by using up to date skills and adhering to
evidence based policies and procedures.
Maintaining
universal infection control procedures.
Providing
a robust Make Every Contact Count (MECC) offer during all patient interactions.
Job
Responsibilities
Independently manage a daily patient list and personal
workload, as directed by the Clinical Managers.
Complete a minimum of 20 housebound patient
visits per standard housebound shift.
Work
independently and as part of a team, under supervision when required.
Communicate routine, complex and sensitive information
to patients and/or their relatives/carers and colleagues.
Handle
difficult conversations (sensitive, language barriers, hostile atmosphere) and
convert complex technical information into easily to understand communication.
Conduct
clinical assessments of patients prior to vaccination
to confirm the patient meets the criteria for safe vaccination, including
previous vaccinations, current health condition, assessment of medical history
and allergies.
Assess each patients capacity to consent to
vaccination, escalating to the Duty Manager on sift with any concerns. This
could include discussing with the family, holders of a Lasting Power of
Attorney and the Registered GP Practice. Vaccinations may also be given under a
Best Interest Decision by the post holder.
Assess the patients readiness for vaccination and
address any concerns the patient may have prior to vaccination.
Provide the patient with verbal and written advice
about any potential adverse reaction and contra-indications.
If a vaccine is contraindicated, give a verbal
explanation and advice when vaccination may be considered in the future.
Ensure the patient understands the vaccination
implications and gain informed, verbal consent to proceed with vaccination.
When
required, work in partnership with patients, relatives and carers
to gain consent for vaccination, treating all with reassurance, empathy, professionalism and handling difficult
conversations.
Appropriate preparation of a variety of vaccinations
in use across the lifespan.
Administer vaccinations to patients who have consented
and been assessed according to the relevant and valid legal framework, Patient
Group Direction.
Record the clinical assessment and outcome on the electronic
patient record or point of care system.
Provide all patients with appropriate MECC information
and interventions as appropriate. Signpost to a variety of NHS and external
organisations as required.
Adhere to the businesss clinical waste and sharps
protocols when disposing of syringes and sharps.
Follow in house,
local and national Infection Control guidelines including the use of PPE when
required.
Responsible for safe use of equipment and resources, ensuring
equipment is maintained and suitable for use.
Complete required local and NHSE documentation at the
end of each vaccination session.
Assist with the monitoring of vaccine and other related stock.
Participate in clinical audits as required.
Undertake additional tasks as required within your general role as
a vaccinator.
