Responsibilities of this job role
To complete comprehensive specialist assessments, including risk
assessments and personalised care plans, ensuring these are regularly reviewed
and developed in partnership with patients and, where appropriate, their
families to support effective clinical outcomes.
To deliver evidence-based therapeutic interventions through individual,
family, and group sessions, while offering specialist advice and clinical
guidance to healthcare professionals involved in patient care.
To act as the key worker for an allocated caseload, coordinating patient
pathways, monitoring progress, and ensuring continuity of care through
effective liaison with all relevant stakeholders.
To produce accurate and timely clinical documentation, including
assessments, treatment reports, progress records, and discharge summaries,
ensuring compliance with local policies, professional standards, and statutory
requirements.
To provide clinical services across a range of settings, including
hospitals, community facilities, schools, residential homes, and patients'
homes, adapting interventions to meet individual needs and circumstances.
To
participate in clinical supervision, safeguarding processes, referral triage,
and specialist practice initiatives, while promoting mental health awareness
and maintaining strong partnerships with healthcare, education, and community
organisations.
