For further information on this role, please see the attached detailed Job Description and Person Specification: • To provide professional and clinical leadership for CYPs with symptom and palliative care needs across care settings. • To act as an expert clinical resource for symptom and palliative care for CYPs, their families and healthcare professionals across care settings. These settings will include home, hospice, GP and POSCUs • To provide support to CYPs and their families where required through community visits to hospital, home or hospice along with relevant allied healthcare professionals as above. • To provide expert clinical care to this group of CYPs and their families. • To demonstrate a high degree of professional autonomy, using clinical judgment, to provide advice. • To participate in the out of hours PATCH on-call rota when deemed competent to do so (and within 12 months of commencing the post) and there is a position available.
