Job responsibilities The role lends itself to applicants who have a strong foundation of knowledge and practical experience in Mental Health or Drug and Alcohol use. We recognise that a hybrid of both skillsets may not be available, and would support the successful candidates in gaining the knowledge required to undertake the role effectively. Delivery of harm minimisation, facilitating safe reduction plans as part of robust care planning, liaison with multiple health care professionals, conducting home visits, and meeting people outside the normal healthcare environment will form part of the role. In addition to this, the successful applicants will also be required to create holistic care plans, identify and manage risks accordingly. Caseload management skills and good organisational skills are essential to supporting the clients treatment journeys. A current driving licence and own transport is essential.
