Key ResponsibilitiesClinical Care: • Provide advanced clinical assessment, diagnosis and management of adults with complex, long-term conditions including frail, elderly and housebound patients in their homes, care homes and community healthcare settings. • Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning. • Work closely with families, carers and social care teams to support older patients with complex needs. Leadership & Coordination: • Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care and voluntary organisations. • Support the development of proactive frailty identification and early intervention within the practice and PCN.
