Community Public Health Nurse (5-19 Service)

• The post holder will work as a Community Public Health Nurse as part of the 5-19 service as part of the wider 0-19 Childrens Services team within Derbyshire Community Health Services NHS Foundation Trust, delivering school based smoking and vaping cessation services to children and young people. • To be work with a defined caseload of children and young people, working closely with the schools and multi-agency partners to improve public health outcomes. • To be part of the multi-disciplinary team including the wider local childrens workforce, working within a public health framework to meet the prioritised health needs of children, young people, and families. • To support students, pre and post registration, and nursing associates when required. • To support the delivery of contacts for a defined caseload of school aged children andableto undertake face to face contacts on a daily basis to respond to client needs. • Identify, assess, and determine client need and where appropriate prescribe from the nurse prescribing formulary or from the British National Formulary (BNF) in your area of competence if you hold the V300 qualification, in accordance with evidence practice based, patient medical history and Trust Policies and Procedures. • To participate in the identification and safe management of vulnerable children and safeguarding families in accordance with the locally agreed Rag rating policy. • To achieve equity of health outcomes for the local population through the provision of an evidence based and needs led service. • Provide targeted interventions for children and young people who meet the referral criteria with emerging needs. • To work closely the lead and wider 5-19 service to review, evaluate and improve services in line with both local and national policy and guidelines. • To utilise a variety of different communication methods including face to face and social media to maximise the reach of key public health messages.
