Crisis and Home Treatment Team Clinician

Operate as part of a multidisciplinary team providing Crisis & Home Treatment for people aged 17 and over. Use own initiative, working autonomously whilst taking responsibility for oneself, quality of work and service provided to patients. Proactively engage in formal and informal education for partnership agencies as well as appropriate students. Actively and creatively engage patients and carers encouraging them to make appropriate use of the mental health and voluntary services available as part of a process of recovery and crisis management. Undertake full psycho-social assessments with those in need of such. Ensuring thorough, precise and timely documentation and dissemination of information in line with Trust policy and procedure.
