Diabetic Specialist Nurse

The successful candidate will: • Undertake comprehensive diabetes reviews, including assessment, monitoring, and treatment planning. • Support patients with medication optimisation, including insulin initiation and titration where appropriate. • Provide education and self-management support to patients and their families. • Monitor and manage patients in line with national and local diabetes guidelines. • Identify and support patients at risk of diabetes-related complications. • Work closely with the multidisciplinary team to ensure coordinated patient care. • Contribute to quality improvement initiatives, audits, and achievement of QOF targets. • Maintain accurate clinical records and ensure effective communication across services.
