The Healthcare Assistant / Care Co-ordinator will work under the direction of the GPs and nursing team to support the delivery of high-quality patient care across the practice group.
The post holder will undertake delegated clinical duties within their competence, support the delivery of preventative healthcare programmes, assist with chronic disease monitoring where appropriate, and contribute to the smooth running of clinical services.
The role also includes care coordination responsibilities, helping to ensure patients receive timely access to appropriate services, supporting continuity of care, and working closely with the wider multidisciplinary team.
As the Practice Group operates from multiple sites, the successful candidate will be expected to work across all locations as required.
Main ResponsibilitiesClinical DutiesUndertake delegated clinical procedures following appropriate training and competency assessment, including:
• Phlebotomy (essential)
• Blood pressure monitoring
• Pulse rate and rhythm
• Temperature measurement
• Height, weight and BMI recording
• Urinalysis
• ECG recording
• Basic wound care and dressings
• Seasonal influenza, RSV, Shingles, Pneumonia vaccinations (where trained or training will be provided)
• Collection and processing of specimens
• Chaperoning clinicians where appropriate
Patient Care
• Deliver compassionate, patient-centred care.
• Promote dignity, privacy and respect.
• Support patients in maintaining and improving their health.
• Provide health promotion advice within own competence.
• Recognise any changes in a patient's condition and escalate concerns appropriately.
Care Co-ordination Responsibilities
• Support patients to access appropriate services both within and outside the practice.
• Assist with care planning and follow-up where appropriate.
• Liaise with community teams and external healthcare providers.
• Support recall programmes and health promotion initiatives.
• Assist in identifying patients requiring reviews or additional support.
• Contribute towards improving patient outcomes through effective coordination of care.
Administrative Duties
• Maintain accurate electronic patient records.
• Complete documentation in accordance with practice policies.
• Manage clinical stock and equipment.
• Ensure consulting rooms are prepared before clinics.
• Clean and maintain equipment in accordance with infection prevention policies.
• Report equipment faults promptly.
Infection Prevention & Control
• Maintain high standards of cleanliness.
• Follow all infection prevention procedures.
• Dispose of clinical waste safely.
• Adhere to national and local IPC guidance.
Communication
• Communicate effectively with patients, relatives and colleagues.
• Work collaboratively within the multidisciplinary team.
• Maintain patient confidentiality at all times.
• Demonstrate professionalism and compassion in all interactions.
Professional Responsibilities
• Work within the limits of own competence.
• Maintain mandatory training.
• Participate in appraisal and personal development.
• Participate in clinical audits and quality improvement activities.
• Adhere to all Practice policies, procedures and NHS guidance.
• Maintain professional registration where applicable.
