Specific Role Responsibilities:
• To ensure that service delivers an excellent standard of care to patients and their families.
• To offer timely and individual direct care based on patient need.
• To communicate fully and effectively with patients and their families or carers offering information and support.
• To communicate with professional colleagues to ensure a safe and needs led care pathway.
• To provide personal care to patient as directed by care plan and document this on EMIS and report any changes back to manager in charge.
• To provide hydration and nutrition as required, and document this in the notes.
• To promote independence and dignity.
• To maintain accurate healthcare records, written and computer based, liaising with community colleagues and in line with Hospice @ Home policies and professional requirements.
• To assist with telephone calls when demand requires
• To maintain collaborative working partnerships with external care agencies, GPs and community teams to facilitate best working practice and service provision.
• To report accidents, incidents and complaints in area of responsibility as per procedure, participating in investigations as required.
• To work to the processes for lone working, role boundaries and patient confidentiality.
• To maintain personal and professional development including Hospice @ Home statutory and mandatory training.
• To look after own health and wellbeing, resulting in excellent attendance.
