To support patients/service users in undertaking activities of daily living within care plans.
To monitor and report changes in patient/service users presentation to the staff member in charge and take appropriate action.
To follow all relevant policies and procedures.
To report issues of concern in line with policy and contribute to any Quality and Safety processes. Communication
To build a rapport with patients/service users and their support network, communicating with them in a way that respects their views.
To use a wide range of communication skills that can be adapted to meet the needs of complex and challenging situations.
To demonstrate effective communication skills with all members of the multi- disciplinary team.
To provide clear, concise, and accurate electronic and paper records of the care given in accordance with agreed policy and standards.
Communicates to patients/service users and relatives compassionately, in a confidential and sensitive professional manner.
To acknowledge and respect language and cultural differences of patients/service users, their support networks and colleagues. Leadership
