Job description

• Working strictly in accordance with specific Hub protocols in the delivery of prescribed programmes of patient care. • Support the clinical team in delivering high-quality, patient-centred care as part of the practice multidisciplinary team. • Undertake a range of Healthcare Assistant duties including phlebotomy, health checks, nursing observations, measurements, and long-term condition reviews. • Provide clinical support including wound care, dressing changes, removal of sutures/clips, urinalysis, ear irrigation, and other delegated clinical tasks. • Support vaccination programmes, health promotion activities, and provide advice to patients on improving health and wellbeing, including smoking cessation support. • Process laboratory samples, complete clinical administration tasks, and assist with the collection and collation of patient data. • Prepare and maintain clinical rooms, equipment, and supplies, ensuring safe standards of care and effective stock management. • Work independently and use initiative while supporting clinicians, undertaking audits where required, and managing delegated patient care activities. • Communicate effectively with patients from a range of cultures and backgrounds, providing compassionate care and maintaining confidentiality. • Identify and escalate clinical or safeguarding concerns appropriately and contribute to maintaining patient safety. • Undertake occasional home visits where required, holding a clean UK driving licence and working flexibly to meet service needs.
