Healthcare Support Worker

To support the Recovery at Home clinical staff service to provide care to people in their own homes to prevent admission/ readmission to hospital over a 24-hour period. To work with the team undertaking roles set out in the patient management care plan. To assist the clinical team in undertaking clinical observations to support diagnosis and treatment. To perform diagnostic tests To schedule own calls as directed to meet needs of the service. Maintain accurate written and electronic information systems, supporting data input onto IT systems.
