About the role We are seeking an experienced Registered Nurse and inspirational leader to join our Integrated Community Care Service as Service Lead. These newly created posts offer a rare opportunity to shape the future of community palliative care across Leeds. Working with the ICC Manager, you will develop services that meet the changing needs of patients, families and the wider health and care system. You will lead and support a multidisciplinary community team, including Clinical Nurse Specialists and Advanced Health Professionals, fostering collaboration, innovation and continuous improvement while developing a skilled, compassionate workforce. Working across organisational boundaries, you will build strong partnerships across health and social care, strengthen integrated working and contribute to local discussions on the future of palliative and end of life care in Leeds. Alongside overseeing safe, effective and responsive day-to-day delivery, you will use insight, data and feedback to drive service development, quality improvement, governance and workforce planning. This is an excellent opportunity for a motivated clinical leader to shape new services, influence care across organisations and make a lasting difference for people affected by life-limiting illness in Leeds.' Working hours are Monday to Friday 09:00 17:00 (however this is flexible) and will include an on call commitment as well as the potential for ad hoc weekend availability.
