Job description

Key Responsibilities: Patient Care • Support patients during medication initiation, titration and monitoring mental health and wellbeing • Identify and escalate clinical risk to appropriate clinical leads, in line with protocols and in line with NICE guidelines • Support patients with understanding their new diagnosis of ADHD, suggested treatment options, lifestyle adjustments and self-management strategies Care Coordination • Work closely with the ADHD service delivery manager and care coordination team to support patient flow through the service and timely delivery of care • Participate in MDT discussions, case reviews and clinical governance meetings • Signpost patients to and liaise with other local services, including GP Surgeries, SPFT and any appropriate voluntary sector partners Record & System Management • Maintain accurate and timely consultations within the clinical records using agreed clinical systems (full training will be provided) • Contribute to outcome tracking, reporting and service evaluation activity as required.
