Group Consultation Coordination
• Coordinate and support the delivery of group consultations for identified patient cohorts
• Identify suitable patients using practice searches and population health data
• Manage invitations, bookings, reminders, and attendance records
• Arrange venues, IT setup, and required materials
• Support the preparation and follow-up of group sessions
Care Coordination and Patient Support
• Work with GPs and the multidisciplinary team to identify and manage a caseload of patients
• Support patients to develop, review, and update personalised care plans
• Provide care navigation and signposting to appropriate services
• Work alongside the Work and Health Coach to support patients on fit notes, including preparation for work-related discussions and referrals
Data Collection and Service Improvement
• Collect and record outcome measures, attendance data, and patient feedback
• Support audits, reporting, and service evaluation activities
Administrative Responsibilities
• Maintain accurate, timely, and confidential patient records in line with information governance requirements
• Manage correspondence, emails, and telephone queries
• Support scheduling of meetings, MDT discussions, and service activities
• Prepare reports, documents, and data extracts
• Use Microsoft Office and clinical systems effectively (training will be provided)
General Responsibilities
• Attend training, supervision, and team meetings
• Contribute to quality improvement activities
• Work in accordance with safeguarding policies and escalate concerns appropriately
