Job description

Act as a role model to colleagues, always seeking to maintain the highest standards of professionalism. Work as part of the Multidisciplinary team across a range of clinical pathways. To manage a caseload of patients with complex dementia and functional mental health needs, using evidence based / patient centred principles to assess, plan, implement and evaluate interventions. Work with patients to identify therapeutic goals through using a Solution focused approach, using specialist mental health tools and care plans. Plan and implement patient centred individual and/or group interventions to achieve therapeutic goals. Maintain robust communication with patients, carers, and other local agencies. Maintain accurate timely clinical records. Support patients and carers to achieve agreed outcomes and goals utilising the recovery model.
