Please refer to the attached job description and person specification for full details of responsibilities. • Review incident reports, sharing and escalating any incidents of concern through appropriate Trust processes, including Incident Safety Panel. • Provide expert advice and information on incident reporting, review & management as well as on wider patient safety matters using a range of communication methods. • Ensure that Statutory Notifications are made to the Care Quality • Commission in a timely manner, providing support and guidance to service managers where required. • Extract learning identified from Learning Response Tools to capture themes and actions. • Produce a monthly Patient Safety bulletin for clinical directorates and other relevant teams to support organisational learning. • Maintain the Patient Safety SharePoint site ensuring key information is up to date and accessible to colleagues across the organisation. • Work with the Patient Safety and Mortality Co-ordinator in maintaining efficient systems and processes to ensure good governance and enable the effective and accurate collection, collation and reporting of routine data and information, producing statistics when required. • Work with the Patient Safety and Mortality Co-ordinator and clinical directorates to ensure that all Patient Safety Incident Investigations (PSII) are reported to the Integrated Care Board (ICB) and Care Quality Commission (CQC) within appropriate timescales. • Work with the Patient Safety and Mortality Co-ordinator and Senior Managers to monitor the implementation of Safety Improvement Plans (SIPs). • Support the Patient Safety and Clinical Risk Manager in the production of reports to the Trust Board, ICB and other internal groups and committees on patient safety and clinical risk.
