Key Responsibilities Clinical Practice The post-holder will: • Work as part of the multidisciplinary team across Huntingdon PCN. • Assess, triage, and clinically manage patients presenting with acute, urgent, and undifferentiated conditions. • Provide same-day clinical assessment and treatment. • Undertake autonomous consultations in surgery, patient's homes, and care home settings. • Manage episodes of care from assessment through diagnosis, treatment, and follow-up. • Recognise red flags, deterioration, and serious underlying pathology, taking appropriate action. • Undertake clinical examinations and investigations as appropriate. • Perform and interpret ECGs. • Collect pathological specimens including blood samples, swabs, and other diagnostic tests within scope of practice. • Prescribe medicines where appropriately qualified and authorised, in accordance with legislation, NICE guidance and local policies. • Provide urgent community assessments and home visits as an alternative to GP home visiting services. • Support management of patients with frailty, long-term conditions, and complex healthcare needs. • Develop and support anticipatory care plans. • Undertake chronic disease monitoring and reviews as required. • Exercise professional judgement and effectively manage clinical risk. • Ensure robust safety-netting and follow-up arrangements are in place. • Refer patients appropriately to primary, secondary, community and voluntary sector services. Caseload Management • Manage own clinical workload and same-day demand. • Prioritise urgent clinical presentations. • Maintain oversight of follow-up requirements. • Track referrals and investigations. • Ensure continuity and safety-netting. • Monitor outcomes and escalate deterioration appropriately. Patient-Centred Care The post-holder will: • Develop therapeutic relationships with patients and carers. • Promote personalised care and shared decision-making. • Empower patients to self-manage their health conditions where appropriate. • Support prevention, wellbeing, and health promotion initiatives. • Address health inequalities and improve access for vulnerable communities. • Signpost patients to social prescribing, community services, and voluntary organisations. • Communicate complex and sensitive information compassionately and effectively. • Ensure patients and carers are fully informed regarding treatment options and care pathways. Multidisciplinary Team Working The post-holder will: • Work collaboratively with GPs, Advanced Practitioners, Practice Nurses, Pharmacists, Physiotherapists, Community Nurses, Mental Health Teams, and Social Prescribers. • Receive referrals and directed patients from triage systems and clinical colleagues. • Attend and actively contribute to MDT meetings and case discussions. • Support integrated working across primary, community and secondary care services. • Act as a professional role model for colleagues and students. • Support mentoring, supervision, and training of junior colleagues where appropriate. Clinical Governance and Quality Improvement The post-holder will: • Maintain accurate, contemporaneous, and legally compliant clinical records. • Use appropriate coding and digital systems to support patient care and reporting. • Participate in audits, quality improvement projects, and service evaluations. • Contribute to incident reviews, significant event analyses, and learning activities. • Support delivery of QOF, Enhanced Services, QIPP and other quality initiatives. • Monitor outcomes and provide data required to demonstrate service effectiveness. • Ensure practice remains aligned with NICE guidance, CQC standards and local pathways. • Participate fully in clinical governance processes.
