Practice Nurse - Diabetes Reviews

• Lead and deliver comprehensive diabetes care clinics for patients with Type 1, Type 2, and pre-diabetes. • Undertake diabetes reviews, including annual assessments and care planning. • Confidently initiate, titrate, and adjust insulin therapy in line with current clinical guidelines. • Support patients with blood glucose monitoring, lifestyle changes, medication management, and self-management strategies. • Interpret clinical results and develop appropriate treatment plans. • Identify and proactively manage patients at risk of diabetes-related complications. • Provide education and support to patients, carers, and clinical colleagues. • Maintain accurate and timely clinical records using the practice's clinical system. • Contribute to quality improvement initiatives and achievement of QOF and local enhanced service targets. • Participate in chronic disease management and health promotion programmes as required.
