Practice Nurse

Job responsibilities • Provide care and management for patients with long-term conditions (e.g. asthma, COPD, diabetes, heart failure, hypertension). • Perform health screenings, cervical smears, vaccinations (including childhood, travel, and campaign programmes). • Support chronic disease management in line with the Quality and Outcomes Framework (QOF) and evidence-based guidelines. • Promote health education, lifestyle advice, and preventative care. • Maintain infection control standards and complete audits as required. • Contribute to the development of integrated, multidisciplinary working across the practice. • Supervise and support HCAs and other team members where needed. • Keep accurate and up-to-date patient records in line with policies and NMC standards. • Participate in annual appraisals, training, and ongoing professional development (CPD).
