Main Duties of the JobThe successful candidate will be responsible for delivering chronic disease management clinics, particularly in diabetes, asthma, COPD, hypertension and cardiovascular disease. They will undertake annual reviews and support patients in the management of their long-term conditions and self-care. The role will include administering childhood and adult immunisations in accordance with national guidance and local protocols, alongside undertaking cervical cytology screening in line with the NHS Cervical Screening Programme. The post holder will also carry out health promotion, screening and NHS Health Checks, provide wound care, injections and general practice nursing duties, and maintain accurate and contemporaneous clinical records using SystmOne. The successful candidate will be expected to recognise, assess and appropriately escalate safeguarding concerns or deteriorating patients, while supporting the practice in achieving QoF and other contractual quality frameworks.
