Practice Nurse

Job Description Job Title - Practice Nurse (Diabetes Special Interest) Location - Windsor, Berkshire Reports To Lead GP Partner/Practice Manager Hours Part-Time 4 sessions per week (approximately 18-20 hours) Contract Permanent Salary £22.00£24.00 per hour (dependent on experience), including paid lunch break. Job Purpose To provide high-quality, evidence-based nursing care within the practice, with a particular focus on diabetes management and long-term condition care. The post holder will lead diabetes services, support patients in achieving optimal health outcomes through education and self-management, and contribute to the delivery of safe, effective and patient-centred care across the practice population. The Practice Nurse will work collaboratively with GPs, pharmacists, healthcare assistants and other members of the multidisciplinary team to deliver preventative, therapeutic and health promotion services. The post holder will also act as the Infection Prevention and Control (IPC) Lead for the practice. Key Responsibilities Diabetes Management • Lead and manage diabetes clinics for patients with Type 2 diabetes and pre-diabetes. • Undertake annual diabetes reviews and ongoing monitoring. • Support effective management of cardiovascular risk factors associated with diabetes. • Develop and implement personalised care plans. • Provide education and support to empower patients to self-manage their condition. • Monitor treatment outcomes and refer appropriately where necessary. Chronic Disease Management • Deliver chronic disease management clinics in accordance with national and local guidelines. • Monitor and manage patients with: • Hypertension • Asthma • Chronic Obstructive Pulmonary Disease (COPD) • Other long-term conditions within scope of competence. • Support achievement of Quality and Outcomes Framework (QOF) targets and enhanced services delivery. Clinical Practice • Administer childhood and adult immunisations and vaccinations. • Undertake cervical cytology screening where qualified. • Carry out NHS Health Checks. • Perform venepuncture and blood sample collection. • Undertake wound assessment and management. • Record and interpret blood pressure measurements. • Perform electrocardiograms (ECGs). • Deliver routine nursing treatments and procedures within professional competence. Health Promotion • Promote healthy lifestyles and disease prevention. • Provide advice and support relating to: • Smoking cessation • Weight management • Physical activity • Healthy eating • Cardiovascular risk reduction • Encourage patient engagement in preventative healthcare programmes. Infection Prevention and Control (IPC) • Act as the practice IPC Lead. • Ensure compliance with infection prevention and control policies and procedures. • Coordinate IPC audits and action plans. • Provide advice and training to practice staff. • Maintain awareness of current legislation, guidance and best practice. Clinical Governance and Quality Improvement • Maintain accurate and contemporaneous clinical records using the practice clinical system. • Participate in clinical audit, quality improvement and service development activities. • Contribute to risk management and significant event reviews where appropriate. • Support implementation of evidence-based practice and continuous improvement initiatives. Team Working • Work collaboratively with GPs, healthcare assistants, pharmacists and other healthcare professionals. • Participate in multidisciplinary team meetings as required. • Communicate effectively with patients, carers and colleagues. • Maintain professional relationships that promote integrated patient care. Professional Responsibilities The post holder will: • Adhere to the NMC Code and professional standards. • Maintain current NMC registration. • Work within the scope of professional competence. • Participate in appraisal and revalidation processes. • Undertake continuing professional development to maintain and enhance knowledge and skills. • Maintain confidentiality in accordance with GDPR and NHS information governance requirements. Person Specification Essential Criteria Qualifications • Registered Nurse with current NMC registration. • Accredited qualification or diploma in Diabetes Care. Experience • Minimum of two years' experience as a Practice Nurse within a UK General Practice setting. • Demonstrable experience managing patients with diabetes in primary care. • Experience of chronic disease management. • Experience administering immunisations and vaccinations. • Experience in venepuncture and wound care. Knowledge and Skills • Sound understanding of diabetes management and current clinical guidelines. • Knowledge of chronic disease management. • Excellent communication and interpersonal skills. • Strong organisational and time management abilities. • Ability to work autonomously and as part of a multidisciplinary team. • Commitment to delivering high-quality, patient-centred care. Desirable Criteria Qualifications • Independent Nurse Prescriber (V300). • Cervical Cytology qualification. Experience • Experience using EMIS Web. • Experience delivering QOF and Enhanced Services. • Experience initiating or managing injectable diabetes therapies. • Experience leading infection prevention and control activities.
