Job description

You will be an experienced Nurse, who, acting within their professional boundaries, will deliver high quality care for patients with a range of chronic diseases in the community setting. You will work with patients and their families to develop personalised goals, manage their condition themselves and lead as independent a life as possible. You will workin partnership with the patient, their family and carers, to develop a personalised plan of care and will be responsible for monitoring and amending plans to meet the changing needs of the patient including referral to partner organisations such as palliative care, social care and social prescribing teams. You will also work as part of a Multi-disciplinary team but will be responsible for a designated caseload of patients registered to the practice. They will demonstrate safe, clinical decision-making and expert care, and work collaboratively to meet the needs of patients and their family.
