Your responsibilities will include but are not limited to: • Providing safe, evidence-based, and patient-centred consultations across a broad range of acute, chronic, and preventative healthcare needs. • Delivering face-to-face, telephone, and digital consultations, ensuring continuity of care wherever possible. • Managing long-term conditions and supporting patients to achieve the best possible health outcomes. • Reviewing pathology results, clinical correspondence, repeat prescribing requests, and other clinical workflows in a timely manner. • Working collaboratively with practice nurses, healthcare assistants, clinical pharmacists, and Primary Care Network colleagues to deliver integrated care. • Participating in clinical governance, quality improvement initiatives, teaching, and continuing professional development. • Contributing to practice meetings and supporting the ongoing development of services to meet the needs of our patients.
