Key Responsibilities The post holder will: • Provide comprehensive general medical services to registered patients. • Undertake face-to-face, telephone and online consultations. • Diagnose and manage acute and chronic illnesses. • Prescribe medications in accordance with local and national guidance. • Manage patients with complex needs, including frailty, long-term conditions and end-of-life care. • Carry out home visits where clinically appropriate. • Interpret and act upon laboratory results, investigations and clinical correspondence. • Assess, diagnose and refer patients to secondary care and community services as appropriate. • Maintain accurate, contemporaneous and complete patient records using the practice clinical system. • Complete associated administration in a timely manner. Clinical Governance and Quality The post holder will: • Work in accordance with GMC guidance and Good Medical Practice. • Participate in clinical governance activities, audits, significant event reviews and quality improvement initiatives. • Support achievement of Quality and Outcomes Framework (QOF), Enhanced Services and other contractual requirements. • Contribute to safeguarding processes for children and vulnerable adults. • Participate in infection prevention and control activities. • Promote patient safety and risk management within the practice. Multidisciplinary Team Working The post holder will: • Work collaboratively with Practice Nurses, Advanced Clinical Practitioners, Pharmacists, Social Prescribers and other members of the team. • Contribute positively to team development and service improvement.
